Signify Health is seeking a Per Diem Clinician (NP/PA) to perform in-home health evaluations, visiting members at their residences. The role is part-time with travel within licensed states and virtual visits as needed.

You will review medical history, perform evaluations, and provide Signify Health Diagnostic & Preventive Services where appropriate. Active unrestricted licenses and board certification are required, with multi-state licenses preferred.

• Visit millions of people nationwide to connect them with the right care.
• Collaborate with Signify Health clinicians and adhere to role requirements.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, DEE, bone density).
