Job summary

Signify Health is seeking a per diem Clinician (NP or PA) to perform in-home health evaluations. The role involves 12 weekly hours, traveling to members' homes, and using digital tools to document findings. It’s a part-time position with benefits eligibility.

Qualifications

• Active, unrestricted licenses in the coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations during 45-minute visits.
• Travel locally to health plan members' homes within licensed states.
• Use iPad and clinical workflow to document visits.
• Provide diagnostic & preventive services as indicated.
