Job summary

Signify Health seeks a Per Diem Clinician (NP or PA) to perform in-home health evaluations across assigned states. Visit duration is about 45 minutes, including medication review, history, and physical assessment. You may conduct virtual visits and participate in additional diagnostic services as needed.

You will join a national network, support health plan members where they live, and travel locally within licensed states while maintaining high-quality, compassionate care.

Qualifications

• Active, unrestricted licenses in applicable states.
• Board certification required.
• Multistate licenses preferred.

Responsibilities

• Conduct in-home health evaluations lasting ~45 minutes.
• Review medications and medical history, perform physical evaluation.
• Provide Signify Health services including DI and preventive tests as indicated.
• Coordinate with health plans for referrals and care connections.
