Job summary

Signify Health, Inc. is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members at their homes across local areas.

You will connect with people, review medications and medical history, perform a physical evaluation, and coordinate with health plans as needed. You will travel locally and within licensed states, conduct virtual visits when required, and support the delivery of diagnostic and preventive services.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Part-time schedule with approximately 29.5 hours per week.

Responsibilities

• Connect with members during home visits and answer health questions.
• Provide in-home health evaluations, review medications and medical history, perform physicals.
• Travel locally to visit health plan members and conduct virtual visits when needed.
• Deliver Signify Health services including diagnostic and preventive services.
