Job summary

Responsibilities

• Bring compassion and clinical excellence to in-home visits.
• Perform health evaluations, review medical history and medications.
• Conduct physical exams and assess health status as needed by plans.
• Coordinate with Signify Health to provide Diagnostic & Preventive Services.
• Travel locally and within licensed states; virtual visits when required.
• Adhere to safety, privacy, and quality standards during visits.
