Job summary

Qualifications

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Perform medication and medical history review and physical assessment.
• Coordinate with health plans and clinicians to bridge gaps in care.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Deliver Signify Health services including diagnostic and preventive screenings.
