Job summary

Signify Health, part of CVS Health, is hiring a Full-Time Clinician (NP or PA) to perform in-home health evaluations, visiting members at their homes and bridging gaps in care. The role uses an iPad and a clinical workflow to guide assessments and engage with patients one-on-one.

Expect 40-hour weeks with travel within licensed states, potential virtual visits, and a comprehensive benefits package including health coverage and retirement plans.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover the following primary locations.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services such as spirometry and bone density screening.
