• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover the following primary locations.

Responsibilities

• Provide in-home health evaluations to patients in their residences.
• Use an iPad and clinical workflow to document visits.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide Signify Health services including Diagnostic & Preventive Services tests when ordered.
