Job summary

Signify Health, part of CVS Health, is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members in their own homes using an iPad and a streamlined clinical workflow. The visit lasts about 45 minutes and focuses on health questions, history review and physical assessment.

Anticipated weekly hours are 40; travel locally and within licensed states is required.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Perform in-home health evaluations for health plan members.
• Review medical history and conduct physical examinations in patients' homes.
• Coordinate with health plans for additional diagnostic tests when ordered.
• Travel locally and within licensed states as needed.
• Document findings using iPad and standard clinical workflow.
