Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for health plan members in their homes. Visits last about 45 minutes and include a meds/history review and physical exam using an iPad workflow.

You will travel locally within licensed states and may conduct virtual visits as needed. A comprehensive benefits package is provided, including medical, dental and vision insurance, PTO, 401K, CEU

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Engage with patients during 45‑minute visits and review meds/history.
• Perform physical evaluations and coordinate with health plans for additional tests as ordered.
• Travel locally within licensed states and conduct virtual visits as needed.
