Job summary

Monogram Health seeks compassionate Advanced Practice Providers (NPs or PAs) to deliver high‑quality in‑home care to patients with multiple chronic conditions. You will work with an integrated multidisciplinary team, providing continuity of care in patients' homes and via telehealth.

The role includes assessments, education, team participation, and thorough documentation in EHR, with a competitive base salary and bonuses, mileage reimbursement, and CEU/CME expense coverage.

Qualifications

• Active and unrestricted NP or PA license.
• Board certification as NP or PA.
• Current DEA certificate or eligibility.
• Minimum of two years direct patient care experience.

Responsibilities

• Provide in-home patient‑focused care to reduce hospitalizations.
• Conduct comprehensive assessments and wellness exams.
• Educate patients and caregivers on disease management and medications.
• Participate in virtual integrated care team meetings.
• Engage in quality assurance and documentation compliance.
• Coordinate care with PCPs and specialists.
