Job summary
Signify Health, part of CVS Health, is seeking a full-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will engage with members in their homes, bridging gaps in care and delivering personalized assessments using an intuitive iPad workflow.
Responsibilities include comprehensive health evaluations, med history reviews, and, when required by health plans, additional diagnostic tests.

- Conduct in-home health evaluations for health plan members.
- Use iPad and clinical workflow to perform medical history review, physical evaluation, and any indicated diagnostic tests.
- Travel locally within licensed states; conduct virtual visits as needed.
