Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations, visiting patients in their own homes to bridge gaps in care. You'll use an iPad and a clinical workflow, with visits lasting about 45 minutes.

You will travel locally within licensed states, conduct additional diagnostic tests as indicated, and join a nationwide network of clinicians dedicated to improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Visit millions of people nationwide to connect them with the right care for their needs.
