Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll use an iPad and a streamlined workflow to conduct assessments, review medical history, perform a physical, and order additional tests if required by the health plan.

40 hours weekly; active licenses and board certification required; travel within licensed

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Visit millions of people nationwide to connect them with the right care.
• Work with Signify Health as a clinician colleague in the specified role.
• Be flexible to travel locally and within licensed states as needed.

• Provide all Signify Health services including Diagnostic & Preventive Services.
