Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members around Jasper, GA. Travel to patients' homes is required, and you will conduct non-invasive physical assessments, review medical history, and document findings accurately.

This role offers a flexible schedule, travel reimbursement, comprehensive training, and dedicated scheduling assistance. A valid NP license, board certification, and BLS are required; prior home-health experience

Qualifications

• Master’s degree required or equivalent experience.
• Active state NP license and national board certification.
• BLS certification; 12 months clinical provider experience preferred.

Responsibilities

• Perform in-home comprehensive physical assessments.
• Review medical history and document findings accurately.
• Provide patient education on wellness and preventive care.
• Coordinate scheduling and travel arrangements.
