Job summary

Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members around Cedartown, GA. This role offers travel to patients' homes, flexible scheduling, and comprehensive home assessment training, with scheduling support and liability insurance provided.

Qualified NPs will have an MSN, active state license, national certification, and BLS, with 12 months of paid advanced clinical provider experience preferred.

Qualifications

• Master's Degree in Nursing or commensurate experience.
• Active State NP license.
• Active National Board Certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
