Job summary

Hueman is seeking qualified Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members around Loganville, GA. This role offers flexible scheduling and travel to patients’ homes, with comprehensive home assessment training and scheduling support.

Responsibilities include non-invasive physicals, history review, and patient education. Active NP license, national board certification, and BLS are required; travel is a core part of the job.

Qualifications

• Master’s degree in nursing required with NP license.
• Active State NP license and national board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.
• Ability to travel to patients' homes for HRAs.

Responsibilities

• Travel to and from patients’ homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety and compliance.
• Provide basic patient education on wellness and preventative care.
