Qualifications

• Active, unrestricted state NP license.
• Board certification: FNP, AGNP, or ANP.
• Reliable personal transportation.
• Available for at least 60 hours/month.

Responsibilities

• Complete pre-scheduled, in-home health assessments.
• Use pre-populated forms for patient assessments.
• Submit completed forms after visits.
