Job summary

Collage Home and Community Rehabilitation Partners in Long Beach, CA is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in patients' homes and communities. PRN positions available to accept patients for the duration of their stay, with interdisciplinary teams developing integrated treatment plans.

Qualifications

• Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification.
