Job summary

Collage Home and Community Rehabilitation Partners is seeking an experienced Occupational Therapist for a PRN position in Chino Hills, CA. Treatments occur in patients' homes and communities, focusing on functional goals and patient-centered care.

The role supports a dynamic, interdisciplinary team delivering personalized rehabilitation in clients' own environments. Qualified candidates have a Bachelor's in OT with CA licensure; a Master’s or certification is a plus.

Qualifications

• Completion of Bachelor's degree in Occupational Therapy and state licensure is required.
• Professional certification/Master's degree in Occupational Therapy is preferred.
• Experience in treatment of neurologically related disorders is strongly preferred.
• Excellent planning, organization, writing and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs, visual and perceptual skills, cognitive needs.
• Address adaptive equipment needs and assistive technology; plan home/community-based therapy.
