Job summary

Triad Home Health Services in Lancaster, OH is seeking a Per Diem Registered Nurse to provide skilled nursing services to Medicare home health patients in their homes. You will perform assessments, implement care plans, educate patients and caregivers.

This role requires Ohio RN license, CPR, reliable transportation, and the ability to work independently in a field-based setting. You will collaborate with clinicians, coordinate care, and ensure timely documentation.

Qualifications

• Graduate of an accredited school of nursing.
• Current Ohio RN license.
• Current CPR certification.
• Valid driver’s license.
• Reliable transportation with current automobile insurance.
• Strong assessment, communication, organizational, and problem-solving skills.
• Ability to work independently in a field-based environment.

Responsibilities

• Nursing assessments and reassessments; update plan of care.
• Coordinate care with physicians, therapists, and other professionals.
• Provide education to patients and caregivers on disease management and self-care techniques.
• Complete accurate and timely clinical documentation and ensure Medicare compliance.
