Job summary

Triad Home Health Services in Ohio is seeking per diem Medicare RN nurses to care for home health clients. You will work independently with a supportive team, focusing on quality patient care and flexible scheduling.

Applicants must be a graduate of an accredited nursing program with current Ohio license and CPR certification; driving and mobility required. We offer per-visit pay, employee benefits, clothing allowance, and referral bonuses.

Qualifications

Responsibilities

• Re-evaluate patient nursing needs and adjust the care plan.
• Implement established care plan and preventive care.
• Counsel patient and family on nursing and related needs.
• Provide health care instructions per assessment and plan of care.
• Identify discharge planning needs and prepare for discharge.
• Document physician communications and coordinate care with the team.

Essential Job Functions/Responsibilities

Patient Care

• 1. Regularly re-evaluates patient nursing needs.
• 2. Implements established care plan.
• 3. Initiates appropriate preventive care and treatments as prescribed by the physician.
• 4. Counsels the patient and family in meeting nursing and related needs.
• 5. Provides health care instructions to the patient as appropriate per assessment and plan of care.
• 6. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Communication

• 1. Document and communicate with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
• 2. Communicates with community health-related persons to coordinate the care plan.

Position Qualifications

• 1. Graduate of an accredited school of nursing.
• 2. Current licensure in Ohio and CPR certification.
• 3. Excellent observation, verbal and written communication skills, problem solving skills.
• 4. Prolonged or considerable walking or standing.
• 5. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
