Qualifications

• Master's Degree from a School of Social Work (MSW)
• Must have, or be in the process of acquiring, ACSW certification
• Licensed Clinical Social Worker (LCSW)
• Two years of experience in a health care setting

Responsibilities

• Assesses the client’s psychosocial status and identifies factors affecting the plan of treatment.
• Contributes to development and implementation of the interdisciplinary care plan.
• Maintains and submits written clinical records including evaluations and daily notes.
• Evaluates client and family response to medical social work intervention and coordinates visits.
• Supervises the social work assistant monthly and participates in discharge planning.
