First Senior Home Health is seeking a Medical Social Worker to provide psychosocial assessments, develop care plans, and support patients in their homes under physician orders. The role requires a Master’s Degree in Social Work, ACSW certification or in progress, and 2 years' experience (1 in healthcare).

Strong communication and high-quality documentation are essential, with CPR certification required. You will evaluate patients, coordinate with the clinical team, and assist in discharge

Qualifications

• Master’s Degree from a School of Social Work approved by CSWE.
• Must have or be in the process of ACSW certification.
• Two years’ healthcare experience preferred, with at least one year in healthcare.
• Criminal background check required.
• Current CPR certification; online CPR not accepted.

Responsabilités

• Provide medical social services to patients in their homes under physician orders.
• Assist in admission by evaluating psychosocial status and home environment.
• Develop and implement interdisciplinary patient care plans.
• Record, monitor, and report changes in emotional and social factors.
• Maintain clinical records including initial evaluation and daily notes.
• Participate in discharge planning and home health coordination.
