Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Must have, or be in the process of acquiring, ACSW certification.
• Two years' experience preferred, with at least one year in a healthcare setting.
• Must have a criminal background check.
• Must have current CPR certification. Online certification is not accepted.

Responsibilities

• Provide medical social services to patients in their homes under physician orders and supervision.
• Assist in admission process by initial evaluation and psychosocial assessment.
• Develop and implement interdisciplinary patient care plans pertaining to medical social work.
• Observe, record, and report changes in emotional and social factors affecting care.
• Consult with attending physician regarding alterations to the plan of treatment.
• Maintain and submit written clinical records, including initial evaluation, care plan, and daily notes.
• Evaluate patient and family responses to social work interventions.
• Participate in discharge planning and weekly scheduling with the Clinical Manager.
• Engage in agency activities, committees, and staff development; supervise social work assistant monthly.
