SA Nurses in Texas seeks an LVN to provide in-home nursing care under RN supervision. You will manage a caseload, ensuring compassionate, patient-centered care and timely documentation in patients' homes.

Ideal candidates hold a state LVN license with 1–2 years in-home care, strong communication, and time-management skills, and the ability to travel between patients daily. WellSky/Kinnser knowledge is a plus.

Qualifications

• State-licensed LVN required
• 1-2 years in-home care experience
• Excellent communication and time management
• Compassionate and friendly demeanor
• Ability to travel to multiple patients per day
• Preferred knowledge of WellSky/Kinnser

Responsabilités

• Under the supervision of a registered nurse, provides quality in-home nursing care per standards and company policies.
• Provides direct and individualized nursing care to assigned patients.
• Ensures quality and safe delivery of home health services.
• Implements the plan of care formulated by a physician and RN.
• Documents accurately and timely according to the plan of care.
• Assesses patient education needs and provides information relevant to diagnosis and plan of care.
• Coordinates home health services with RN and clinical management.
• Assists the patient in learning appropriate self-care activities.
