Job summary

SA Nurses is hiring Licensed Vocational Nurses (LVNs) to provide in-home care to patients of various ages. You will manage a heavy caseload with strong time management and clear communication while delivering compassionate, patient-centered care.

Under supervision of a registered nurse, you will follow the plan of care, document accurately, and coordinate with clinical management. Willingness to travel to multiple patients per day is required.

Qualifications

• State-registered nurse license required.
• 1–2 years of experience in in-home care.
• Excellent communication and time-management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Preferred knowledge of Wellsky/Kinnser.

Responsibilities

• Under supervision of a registered nurse, provides quality care to patients in home health care.
• Delivers direct and individualized nursing care according to plan of care.
• Ensures safe, high-quality delivery of home health services.
• Implements a physician and RN plan of care.
• Documents accurately and timely per plan of care.
• Educates patients, clients and families as needed.
• Maintains a safe environment and follows clinical standards.
• Coordinates home health services with RN and management.
• Assists patients in learning self-care activities.
• Uses equipment and supplies effectively.
• Documents and reports activity as required.
