Ideal candidates have 1–2 years of in-home care experience, strong communication and time-management skills, and a compassionate, patient-centered approach.

Qualifikationen

• Active state RN license required.
• 1–2 years of in-home care experience.
• Excellent communication and time-management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Knowledge of WellSky/Kinnser preferred.

Aufgaben

• Under supervision of an RN, provides quality care in the patient's home in compliance with nursing standards and policies.
• Provides direct and individualized nursing care based on standards under supervision.
• Ensures quality and safe delivery of home health services to patients and families.
• Implements a physician and RN-formulated plan of care.
• Provides accurate and timely documentation per the plan of care.
• Educates patient and family about diagnosis and plan of care.
• Maintains a safe working environment per clinical standards.
• Coordinates home health services with RN and management.
• Assists the patient in learning appropriate self-care activities.
• Uses equipment and supplies effectively and reports activity.
