UPMC Health Plan is seeking Service Coordinators in Blair and Cambria Counties to join the Community HealthChoices team. This hybrid role requires daily community travel to assess and support members in their homes and communities.

Qualified candidates will have a strong background in case management and care coordination, with experience conducting in-home assessments and visits for adults with complex medical, behavioral, and social needs.

Responsabilidades

• Establish and build strong relationships with internal team members and partner providers to coordinate community resources.
• Manage an active caseload based on state-mandated ratios and track participant progress for reporting and compliance.
• Prepare reports and update service plans; ensure comprehensive assessments are completed within required time frames.
• Lead the Person-Centered Service Planning (PCSP) process and assist Members in obtaining HCBS services for independent living.
• Identify and assist Members in accessing LTSS and related services, and coordinate non-Medicaid supports.
• Inform Members about LTSS options, DHS Fair Hearing rights, and assist with fair hearing requests when needed.
• Collect information on Member preferences and goals to inform PCSPs and reevaluate level of care as needed.
• Coordinate with individuals and entities essential to physical and behavioral care delivery; ensure HIPAA compliance.
• Willingness to work in the field at least 75% of the time and to work independently in a virtual setting.
