UPMC Health Plan is seeking Service Coordinators in Blair and Cambria Counties to join the Community HealthChoices team. This hybrid role requires daily community travel and strong case management skills to support LTSS and HCBS services.

The role focuses on coordinating care for adults with complex medical, behavioral, and social needs, ensuring safe independent living in home and Community settings.

Qualifikationen

• Strong background in case management and care coordination.
• Experience conducting in-home assessments and visits.
• Ability to work with adults with complex medical, behavioral, and social needs.

Aufgaben

• Establish and foster relationships with internal team members and partner providers.
• Educate on and coordinate community resources across medical, behavioral, and social services.
• Manage an active caseload and track progress for OPS reporting.
• Prepare reports and update service plans according to governing bodies.
• Lead PCSP process and oversee implementation; assist Members with HCBS services.
• Identify and assist Members in accessing LTSS and Medical Assistance.
• Provide information and facilitate access to LTSS and other services.
• Inform Members about LTSS, assessments, and DHS Fair Hearing rights.
• Collect Member preferences, strengths, and goals for PCSP development.
• Ensure LTSS eligibility activities and explore service coverage sources.
• Coordinate with others to ensure seamless delivery; comply with HIPAA; field work 75%.
