Job summary

Maxim Healthcare Services is seeking a dedicated Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a patient in their home in the Scranton, PA area. You will deliver personalized care, build strong relationships with the patient and family, and work with a multidisciplinary team.

This role requires an active PA LPN license and CPR certification, with the flexibility to adapt to a home environment and schedule.

Qualifications

• Current and unrestricted Pennsylvania LPN license.
• Current CPR certification.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently in a home care environment.
• Private duty/home health or pediatric nursing preferred but not required.

Responsibilities

• Provide skilled nursing care in accordance with physician orders and care plan.
• Monitor and assess patient health status.
• Administer medications, treatments, and therapies as prescribed.
• Assist with activities of daily living (ADLs) as needed.
• Measure and document vital signs and progress.
• Observe and report changes to the clinical team.
• Maintain nursing documentation accurately and timely.
• Educate patients and families on care and health management.
• Ensure patient safety and infection control standards.
• Collaborate with physicians, caregivers and the health care team.
