Job summary

Perfect Care Nursing is seeking a licensed practical nurse (LPN) in Georgia to deliver high-quality home care. You will develop and regularly reassess each patient’s service plan under medical direction and coordinate with physicians and families to ensure safety and comfort.

The role requires strong communication, documentation, and the ability to adapt to patient needs in a homecare setting. Weekend/holiday coverage may be included as part of the schedule.

Qualifications

• Graduate of an accredited nursing program.
• Georgia LPN license required and CPR certification.
• Strong communication and patient-centered care skills.

Responsibilities

• Develop and implement the service plan for each patient.
• Participate in in-service training and educate other staff.
• Observe, report, and document patient conditions and treatment responses.
• Maintain patient care notes and progress reports.
• Coordinate with other agencies and health care providers as needed.
• Counsel patients and families on nursing needs and care options.
• Supervise caregivers and write clear care instructions.
