Job summary

- Current, unrestricted LPN licensure in the state of practice.
- Current CPR certification.
- Reliable vehicle or access to transportation.
- Ability to work with diverse patient populations in home settings.
Responsibilities
- Provides high-quality clinical nursing services in patients' homes under the plan of care and in coordination with the health care team.
- Documents patient visits in EMR and EVV according to regulatory and payer requirements.
- Coordinates patient care and communicates changes in condition to the team and family.
