Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders. You will document progress notes and coordinate other patient care activities within the service area.

You will work under supervision of a Registered Nurse or supervisor, delivering high quality care and participating in the agency's care coordination processes.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred (within 90 days).
• Ability to work flexible hours to meet client needs.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides high quality clinical services within scope of practice per plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician, and team.
• Function in diverse home care settings regardless of client background.
