Zusammenfassung

Easterseals Southern California seeks a dedicated home care assistant to support participants in their Lancaster, CA homes. You will plan daily activities to develop living skills, promote independence, and facilitate community integration in line with ISP/IPP goals.

Responsibilities include 1:1 personal care, thorough documentation, ISP/IPP meetings, and mandatory CPI/PART/MAB training, with local travel as needed.

Qualifikationen

• Typically requires a High School Diploma or GED, or equivalent.
• CPR and First Aid certifications are required.
• CPI/PART/MAB training or equivalent handling challenging behavior is required.

Aufgaben

• Plans and prepares a weekly schedule of activities aligned to ISP/IPP goals.
• Conducts daily activities at the participant's home to build living skills and independence.
• Provides 1:1 personal care including help with eating, hygiene, and bedtime routines.
• Maintains documentation for ISP/IPP, evaluations, and case notes.
• Attends ISP/IPP meetings and supports service plan development.
• Completes required training on schedule and performs other duties as assigned.
