Easterseals Southern California seeks a Home Care Support Specialist to assist a participant in their home with daily living skills and community integration. Responsibilities include planning activities, providing personal care, and maintaining ISP/IPP documentation.

The role requires 1–2 years of experience or equivalent training, a high school diploma or GED, CPI/PART/MAB training, and CPR/First Aid certification. Local travel and occasional outdoor work are expected.

Formación

• Experience working with individuals in behavioral management programs is preferred.
• 1–2 years of relevant experience or equivalent combination of training and education.

Responsabilidades

• Plans and prepares a weekly schedule of activities to develop living skills.
• Conducts daily activities at the participant’s home to build independence and community integration.
• Provides 1:1 personal care including eating, hygiene, and bedtime preparation.
• Maintains documentation such as ISP/IPP, evaluations, and case notes.
• Assists in assessments, planning, and evaluation processes; participates in ISP/IPP meetings.
• Completes required training hours and performs other duties as assigned.
• Carrying/Lifting: 0–50 lbs; standing, walking, and repetitive motions as described.
