Job summary

Collage Home and Community Rehabilitation Partners in Fallbrook, CA, is seeking experienced Licensed Clinical Social Workers (LCSW) to provide patient-centered care in patients' homes and communities. PRN positions are available for clinicians who will accept patients for the duration of their stay.

Our interdisciplinary teams develop integrated treatment plans for neurological and complex orthopaedic conditions, using evidence-based approaches in home settings to achieve durable outcomes.

Qualifications

• Master's in Social Work and state licensure as an LCSW required.
• Excellent planning, organization, writing and time management skills required.
• Ability to communicate effectively with families, professionals and co-workers.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans for adjustment to disability and resource linkage.
• Build and implement patient-centered home- and community-based treatment plans.
• Collaborate with interdisciplinary teams to address functional goals in living environments.
