Job summary

BayCare HomeCare in Largo, FL is seeking an experienced Home Health Infusion Field RN to deliver hands-on in-home nursing care. This role focuses on infusion therapies with a strong emphasis on patient education and independence, avoiding hospital intensity.

You will lead a multidisciplinary team, coordinate visits with the supervisor, and ensure visits align with physician orders; weekend coverage is occasional, and a Florida driver's license is required.

Qualifications

• BLS certification required.
• RN license required.
• Florida driver's license required within 30 days of hire.
• Associate's degree or Nursing Diploma required.
• Minimum 1 year RN or 1 year LPN experience.
• BSN preferred; 1 year home care preferred.

Responsibilities

• Oversee the activities of the multi-disciplinary team providing care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of schedule changes and communicates caseload and patient care issues to Home Care Supervisor.
