Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments for Medicare and Medicaid members in and around Doylestown, OH. This role offers the freedom to balance professional goals with a flexible schedule and community connections outside a typical clinic.

Qualified NPs will perform non-invasive assessments, review histories, and document thoroughly while traveling to patients' homes.

Qualifications

• Master’s degree or commensurate NP experience with license.
• Active NP license in the state and national certification.
• Current BLS certification.
• Willingness to travel to patients’ homes.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately.
• Conduct medical reconciliation to ensure safety.
• Provide basic wellness education to patients.
