Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Saint Marys, PA. This role offers flexible scheduling and the chance to balance professional goals with personal preferences.

Qualified NPs hold an active state license, national board certification, and a Master’s degree in nursing. Travel to patients’ homes is required, with liability insurance options and scheduling support provided by Hueman.

Qualifications

• Master’s degree in nursing or commensurate experience with NP licensure.
• Active state NP license with national board certification.
• Current BLS certification and willingness to travel to patients’ homes.

Responsibilities

• Travel to and from patients’ homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic wellness education and preventive care guidance.
