Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Lititz, PA. This role offers a flexible schedule and the freedom to tailor your workload while making an impact in the community.

Qualified NPs will travel to patients' homes to conduct non-invasive assessments, review histories, and document findings. A master’s degree, active NP license, and board certification are required.

Qualifications

• Master’s degree in nursing required.
• Active NP license in the state.
• Board-certified NP with current credentials.
• BLS certification required.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Perform medical reconciliation to ensure safety and compliance.
• Educate patients on wellness and preventive care.
