Job summary

Hueman seeks licensed Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members in and around Newnan, GA. The role offers flexible scheduling and the opportunity to work outside a traditional clinic setting.

Providers will travel to patients' homes, conduct non-invasive assessments, review histories, and document findings. BLS certification and active NP license are required, with board certification preferred.

Qualifications

• Must hold a Master’s Degree or commensurate NP experience.
• Active State NP license is required.
• National board certification for NP required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
