Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Lithonia, GA. This position requires travel to patients’ homes and offers a flexible schedule designed to fit your lifestyle and goals.

Qualified NPs will perform comprehensive non-invasive assessments, review medical histories, and document findings accurately while providing basic wellness education. NP licensing and board certification are required.

Qualifications

• Master’s degree or commensurate experience in advanced nursing practice.
• Active State NP license is required.
• Active National board certification is required.
• Current BLS certification is required.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
