Northeast Healthcare Recruitment, Inc. (NEHCR) seeks experienced Nurse Practitioners to perform in-home Health Risk Assessments for patients across Jackson County, Oregon.

This part-time role offers autonomy with set-your-own-hours scheduling and visits available 7 days a week, minimum 8–10 hours weekly. Compensation includes $130 per completed assessment plus mileage reimbursement and no-show pay, providing meaningful supplemental income for skilled clinicians.

Qualifications

• Active Nurse Practitioner license in Oregon.
• Experience with Health Risk Assessments (HRA) preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage schedule efficiently.
• Reliable transportation for in-home visits.

Responsibilities

• Conduct comprehensive in-home Health Risk Assessments (HRAs).
• Identify and document acute and chronic medical conditions.
• Obtain vital signs and calculate BMI.
• Review medications and pharmacological therapy.
• Perform a physical examination.
• Evaluate patients’ physical, mental, and social health needs.
• Educate and counsel patients and their families on health conditions and preventative care.
