Job summary

Hueman is actively recruiting Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Jarrell, TX. This role offers flexible scheduling and the freedom to align work with your lifestyle while contributing to community care.

Travel to patients' homes, conduct non-invasive exams, review histories, ensure accurate documentation, and provide basic wellness education as part of comprehensive risk assessment.

Qualifications

• Master’s degree in Nursing or commensurate experience with NP licensure.
• Active State NP license and current national board certification.
• Valid BLS certification; 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients’ homes for HRAs.
• Perform comprehensive non-invasive physical assessments.
• Review medical history and ensure documentation is accurate and timely.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive care.
