Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members around Southbridge, MA. This role offers a flexible schedule, travel reimbursement, and dedicated scheduling support, with comprehensive training provided.

Nurse Practitioners will conduct non-invasive assessments, review medical history, and document findings. The position emphasizes patient connection without in-home prescribing, ensuring safe, person-centered care.

Qualifications

• Master’s Degree in Nursing (NP) or commensurate experience.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
