Job summary

Hueman is hiring Nurse Practitioners to complete in-home Health Risk Assessments for Medicare and Medicaid members in and around Princeton, IN. This role offers a flexible schedule designed to fit your lifestyle while delivering meaningful community health work.

As an HRA NP, you will travel to patients’ homes, perform non-invasive assessments, review medical histories, and provide basic wellness education.

Qualifications

• Master’s Degree in Nursing or commensurate experience and completion of NP license.
• Active State NP license.
• Active National Board certification.
• Current BLS certification.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and adherence.
