Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Pine Mountain, GA. This role offers a flexible schedule tailored to your preferences and includes travel reimbursement.

Qualified NPs will perform non-invasive assessments, review medical histories, and provide basic wellness education while maintaining thorough documentation and patient safety.

Qualifications

• Master's degree in Nursing (NP) or commensurate experience with NP licensure.
• Active State NP license and active national board certification.
• Current BLS certification and 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document promptly and accurately.
• Provide basic wellness education and ensure patient safety.
