Job summary

Northeast Healthcare Recruitment, Inc. is seeking experienced Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) across Deschutes County, Oregon. This part-time role offers flexible hours and the opportunity to earn income through completed assessments.

Responsibilities include conducting HRAs, documenting conditions, taking vitals, reviewing medications, and educating patients. Must have an active Oregon NP license and reliable transportation for home visits.

Qualifications

• Active Nurse Practitioner license in Oregon.
• Experience with Health Risk Assessments (HRA) preferred.
• Strong clinical assessment and documentation skills.
• Able to work independently and manage schedule efficiently.

Responsibilities

• Perform in-home Health Risk Assessments (HRAs).
• Identify and document acute and chronic conditions.
• Record vitals and BMI; review medications.
• Conduct physical examinations and assess overall health.
• Educate patients and families on health conditions and prevention.
