Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Deschutes County, Oregon. This role offers flexible scheduling and the freedom to balance personal goals with meaningful patient care.

Travel to patients' homes, perform non-invasive assessments, review medical histories, and document findings. Active NP license, board certification, and BLS are required; 12 months of paid advanced clinical provider

Qualifications

• Master’s degree in Nursing or commensurate NP experience.
• Active State NP license and board certification.
• BLS certification is required.
• 12 months of paid advanced clinical provider experience preferred.
• Strong documentation and communication skills.

Responsibilities

• Travel to and from patients’ homes for assessments.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure safety and compliance.
• Provide basic patient education on wellness and prevention.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients’ homes.

• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements:

• Master’s Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
